What is shockwave therapy? How does it speed up injury recovery?

I hear some version of this story a lot: "It gets better when I rest, but the second I'm active again, it's right back." If that's been your experience with a tendon or soft tissue injury, shockwave therapy might be the piece that's been missing. Here's what it actually is, how it works, and why I reach for it in certain cases.

How Shockwave Therapy Works

Shockwave therapy sends acoustic pressure waves through the skin into the injured tissue. Those waves boost blood flow, kick-start your body's natural healing response, and help break down the chronic scar tissue and calcium buildup that can form in tendons after months (or years) of repeated strain.

Think of it this way: it jumpstarts healing in tissue that's basically stalled out and stopped repairing itself.

What It Actually Helps With

I use shockwave most often for chronic, overuse-type injuries — the kind that don't respond well to rest alone because the tissue needs a stimulus to remodel and heal, not just time off. That includes things like:

  • Plantar fasciitis

  • Achilles tendinopathy

  • Tennis elbow and golfer's elbow

  • Rotator cuff tendinopathy

  • Patellar tendinopathy (jumper's knee)

  • Hip and IT band pain

What Does It Actually Feel Like?

Most of my patients describe it as a fast tapping or pulsing sensation right at the treatment site. The first session or two can feel pretty intense, especially over an area that's already sensitive, but it's generally well tolerated and you can go right back to your day afterward — no downtime needed. Most people notice a difference within a few sessions, though I usually recommend a full course of 6 to 10 treatments, spaced about 2 to 3 days apart, to get lasting results.

Why I Don't Use It Alone

Shockwave is a powerful tool, but I've never seen it work as well as a standalone fix. I almost always pair it with targeted rehab exercises, and when it makes sense, BFR training to rebuild strength in that tissue as it heals. The goal isn't just getting you out of pain — it's making sure you come out of this stronger and more resilient than you were before the injury.
